The Problem
The healthcare industry struggled to deliver personalized and comprehensive care due to the lack of detailed care plans. Providers didn’t have a streamlined way to access patient health insights, making it difficult to identify care gaps and provide tailored treatment.
Who It Affected
Patients receiving generic, non-personalized care, leading to frustration and dissatisfaction with their medical experiences.
Why It Mattered
Without clear care plans, providers were unable to offer exceptional, individualized care. This lack of personalization resulted in missed treatment opportunities, lower patient trust, and ineffective clinical visits.
The Solution
To address the challenge, we built the Care Plan Chrome Extension—an AI-powered tool that acts as a dynamic checklist for providers during patient consultations. It integrates with EMR systems and offers real-time insights, enabling doctors to deliver personalized, data-driven care.

My Design Process
Research & Insights
Since I didn’t have direct access to end-users, I gathered insights from stakeholders to understand the extension’s use cases.
I studied the main Care Plan product to ensure the extension aligned with its core functionality and maintained consistency.
Conducted secondary research on existing EMR extensions to identify best practices for usability and performance.
Ideation & Wireframe
I started with low-fidelity wireframes, sketching out the layout and content placement.
Incorporated stakeholder feedback to refine the structure, ensuring the extension was intuitive and easy to navigate.
Iterated on the design to create a responsive layout suitable for Chrome extensions.
Design & Prototyping
Created high-fidelity designs in Figma, ensuring a clean and organized UI with consistent branding.
Developed interactive prototypes that mirrored real-world usage, helping stakeholders visualize the final product.
Added micro-interactions to enhance user experience, including visual indicators for active care plans and points earned.
Usability Testing & Iterations
Based on stakeholder feedback, I designed Version 2 to include a concise overview of the care plan, categorized into Conditions, Labs, Imaging, and Vaccinations. Added a toggle feature, allowing providers to switch between the detailed checklist view and the summary view for flexibility.
